Initial Clinical Assessment: $200
- Includes a video evaluation and, if approved, an official letter from a licensed clinician.
Renewal Clinical Assessment: $150
- Covers video reassessment and, if approved, an updated official letter from a licensed clinician.
Additional Forms: $65
- Fee for completing supplementary documentation.
